ARISE’s Medicaid Service Coordination is designed to help people who have a developmental disability increase their ability to live a meaningful, independent life.
A Medicaid Service Coordinator works with you to develop an Individualized Service Plan (ISP) that identifies the supports you need to maximize your capabilities and reach your goals. Your Medicaid Service Coordinator also plays an active role in helping you access community resources by communicating with service providers and advocating for quality services.
ARISE’s Medicaid Service Coordination program has certain eligibility requirements.
